• Form 17 - Statement of Jurisdiction *
• Parenting Plan (cases with minor children)
• Form 104 (CV105) - Statement of Marital and Non-Marital Property
• Form 105 (CV100) - Statement of Income and Expense
• Certificate of COPE Attendance (cases with minor children)
• Certificate of Dissolution of Marriage (Department of Health form) *
• Decree of Dissolution

When a case is filed, the Circuit Clerk issues a case number. You can see information about the case at Case.net. Additional forms are available on this site and at www.courts.mo.gov.
